Colonia quickly took the lead with a goal from Nicolas Milessi in the first few minutes of the game.
Peñarol got the equalizer in the 27th minute after picking up the rebound from a Luis Aguiar shot.
A penalty kick goal in the 79th minute by Alejandro Villoldo gave the victory and the first professional title to the team from Colonia de Sacramento, 170 km west of Montevideo.
The win also broke Peñarol's undefeated streak in their new stadium opened earlier this year.
The road to the title for Plaza Colonia was open on Saturday when Nacional fell 2-0 to Rentistas with goals from Matias Mier.
